YOU MUST COMPLETE ONLINE EVALUATIONS IN ORDER TO OBTAIN YOUR CME CERTIFICATE 

ONLINE DIRECTIONS

1. Log-in to the Conference Attendee Portal

      Go to: https://tinyurl.com/ITSVA2020

 Log-in using the email address you used to register for the conference

      Select ‘Evaluations: Click here to view’

2. Evaluate the Conference

     Click on ‘Conference Evaluation’ and complete the online form    

3. Evaluate the Sessions

     Select ‘Individual Session Evaluation’

     Evaluate only the sessions you attended

     Remember to click ‘submit’ at the bottom of the page

4. Print Your Own Certificate

    Select ‘Certificates: Click here to view’ to print your conference CME Certificate 

IMPORTANT: You must complete both the Conference Evaluation and your Session Evaluations in order to print your CME certificate

DEADLINE FOR SUBMITTING EVALUATIONS IS December 18, 2020.
